Becoming a CNA is a challenging and rewarding career for compassionate and hard-working individuals. One of the many reasons people decide to become CNAs is that this career path enables people to practice their love for caring for others. Our Nurse Aide program has been skillfully designed to provide our students with both classroom instruction and hands-on clinical training. It also provides you with the knowledge, skills, and practice needed to pursue a new career.

Please review the schedules carefully and choose the best option for you. The Class Code is located at the top of the schedule, and you will need it to complete your application. Once you have completed the application, you will also need to provide payment information. You will receive an email within 24 hours from Registration@QualityCNATraining.com or Tanya@QualityCNATraining.com. Please check your spam folder if you don’t receive an email with 24 business hours. 

To become a certified nursing assistant (CNA), you must submit specific documents and forms of identification.

A valid driver’s license or passport – This is needed on your final exam day. Expired documents are not accepted.

A Background Information Disclosure form – This will be emailed to you and will need to be completed and sent back.

Notification of policies form – This will need to be read, signed, and returned via email with your background check form.

Fitchburg Students must submit their Childhood Immunization Record – You can access your childhood immunization records on this website. https://www.dhfswir.org/PR/clientSearch.do?language=en

Special Accommodations: Students requiring special accommodations, including ESL students, should email us specifying their needs. We will provide accommodations based on the requirements provide to us by DHS.

EIP Students: Individuals who have an IEP and are requesting special accommodations will need to complete the ADA accommodations form and submit a copy to QCNAT and to DHS. COVID-19 Vaccination Records – If you have been vaccinated against COVID-19, please email us a copy of your  vaccination card.

Covid-19 Exemption Requests – Please email us a letter outlining why you are requesting an exemption. Exemptions are approved by the clinical sites.

Medical Release – Students who are pregnant or have a history of back issues will need to provide a medical release from your physician. This medical release should confirm that you do not have any physical limitations that would prevent you from completing your training. We will provide the necessary accommodation based on the requirements provided to us by DHS.
