Once you've submitted your application and are enrolled in one of our classes, you will need to complete a tuberculosis (TB) test with a licensed physician. Click the button below to download the TB test form, and then print it off and bring it with you to your TB test appointment.
Once you are finished with the test, bring the completed form back to class by the date listed on your class schedule.
If you are not quite ready to apply or need help covering the cost of our course, we encourage you to complete our demographics form. Using this form, we can try to match you with potential funding sources that may be able to reduce or eliminate your out-of-pocket expenses for the course.
